Looks like a California extra-cost fuel nozzle. I have net been to the left coast for a long time, but it seems like I was never able to get any thing like the pictured flow of that anti-smog collar if firmly engaged--enough to tip over a gas can that you are trying to fill.
California... that explains some of it, sure ;-)
Must be a posthumous photo. She's bound to have blown herself up by now. Evolution in action. Rarely seen, but always understood.
I thought about another angle: nothing will clean your car better than a dose of high octane.
And take the paint off---good prep for a new paint job.
